| PolicyNumber | The unique number assigned to the policy, or submission, being referenced. If required for self-insurance, the self-insured license or contract number. |
| PolicyTerminatedCd | A code indicating if this policy was terminated for any reason within a specified period of time. The indication, in general, refers to whether or not coverage has been terminated in the past. If so, additional information regarding is required. This element uses the ReasonTerminated code list. |
